Inspector Notes

An unannounced inspection was initiated on and completed on 04/21/2025 in response to a complaint received by the licensing office on 04/14/2025 relating to allegations of care and supervision of children. The inspector reviewed 1 children?s record and the record of attendance on-site. This inspection included tour of the family day home, observations of the programming space and interview of staff.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-560-A
A caregiver shall provide direct care and supervision of each child at all times. Direct care and supervision of each child includes awareness of and including being near enough to intervene if needed.

On 03/26/2025, Staff #1 admitted to laying Child #1 (5 months) on a changing table while supporting the child's head while being bottle fed. Staff #1 safety strapped Child #1 on the changing table and walked away approximately 10 feet to unlock the front door.
Plan of Correction: This will never occur again. All infants who can not hold their own bottles will be held by staff while feeding them. Staff will never leave a child unattended on a diaper changing table.
