Inspector Notes
An unannounced renewal inspection was conducted today between the hours of approximately 11:55 a.m and 1:25 p.m. There were 10 children present, between the ages of 4 months and 3 years, with the Provider and 2 assistants for a total of 31 points within the supervision guidelines. The children were observed preparing for lunch and napping. Positive interactions were observed during this inspection. A complete inspection of the physical plant, playground, children and caregiver records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations
7Evidence: Caregiver #3 (start date: 11/25/2020) currently resides out of state. The provider stated that a search of the child abuse and neglect registry from that state had not been conducted.
Evidence: The provider did not have documentation of a physical examination for Child #3 (start date: 01/06/2022).
Evidence: A toaster oven, located in the kitchen, was plugged in while not being used.
Evidence: The infant seat safety strap was not used and securely fastened as Child #5 sat in it.
Evidence:
1. Child #1?s record did not contain a telephone number of the child?s physician.
2. Child #4?s record did not contain a name, address and telephone of the child?s physician.
3. Child #1?s record did not contain an address for emergency contact #1 and an address and phone number for emergency contact #2.
4. Child #4?s record did not contain an address for emergency contact #2.
5. Child #1?s record did not contain the name and policy number of the child?s medical insurance.
6. Child #4?s record did not contain the policy number of the child?s medical insurance.
Evidence:
1. The provider did not have a written authorization form for Child #6?s sunscreen.
2. The provider did not have a written authorization form for Child #7?s diaper cream.
Evidence: The provider did not have documentation of immunizations for Child #1 (start date: 08/01/2022).
