The center shall implement a monthly practice evacuation drill.

Interviews with staff supported the complaint that fire drills were not being conducted monthly as required but were being documented as being conducted.

All staff interviewed stated that it had been anywhere between 8 months and a year when the last drill was practiced. The LI also reviewed the drill log and it had been completed through September 2025 and staff verified in interviews that they were just documented but not practiced.
Plan of Correction: Drills will be conducted monthly as required.
