Inspector Notes

An unannounced monitoring inspection was conducted today between the hours of 10:15 am through 11:15 am. There were a total of four children (9 points) observed with two caregivers. A total of three children's files and five staff/household member's files were reviewed. The children were engaged in the following: toy play, outside time , snack, hand washing, and other organized activities. Areas of non-compliance are identified in the violation notice. Standard 8VAC20-800-100-A
Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #1's (Start date-05/23/22) record did not contain documentation of a completed physical.
2. Child #2's (Start date-08/02/21) record did not contain documentation of a completed physical.
3. Staff #1 confirmed that the physicals were not obtained.
Plan of Correction: The provider will ask the parents for a copy of the physical examination.
Standard 8VAC20-800-60-A
Based on record review, children's records did not contain required information.
Evidence:
1. Child #1's record (Start date-05/23/22) did not contain the name of the custodial parent's place of employment's name, address, name and policy number of the child's medical insurance.
Plan of Correction: The provider will ask the parents for the missing information and document the record.
