Inspector Notes
An unannounced renewal inspection was conducted on December 8, 2023 from 9:30am-10:30am. At the time of entrance, one child (2pts) was in care with one caregiver. The inspector reviewed one child's record and five caregiver/household members record. The child was observed having snack, playing with toys, playdough, and enjoying music. Areas of non-compliance are identified on the violation notice. Repeat violations were found and are identified within this report.
Plans of correction were obtained from the provider.

3Evidence:
1. Caregiver #1 did not update criminal history record check report every five years. Documentation of the last criminal history check was dated 11/19/2018.
2. Household Member #1did not update criminal history check report every five years. Documentation of the last criminal history check was dated 12/04/2018.
3. Household Member #2 did not update criminal history check report every five years. Documentation of the last criminal history check was dated 11/29/2018.
4. Caregiver #1 confirmed that a criminal history check had not been conducted at the time of the inspection.
Evidence:
1. Caregiver #1 did not complete a minimum of sixteen clock hours of annual training. Caregiver #1 completed 8 hours of the required 16 hours of training.
Based on record review, children's records did not contain required information.
Evidence:
1. Child #1's record (Start date-11/1/2021) did not have documentation of an annual review of the child's emergency contact information.
