Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on December 9, 2024 as a part of the licensure period. The on-site inspection began at 10:30am and ended at 11:45am. The inspector reviewed compliance in the areas listed above.There were five children present and one caregiver; the point total was 14. The inspector reviewed five children?s records and five caregiver/household member(s) records on-site. This inspection included document review , tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
3The provider shall obtain documentation of a physical examination prior to the child's attendance or within 30 days after the first day of attendance.
There were no physical examination records for Child #1 and Child #5 who had been in care for over 60 days.
Children's records are required to contain the address of two emergency contacts and the policy number of the child's medical insurance.
The record for Child #1 was missing the addresses of two emergency contacts and the policy number of the child's medical insurance. Child #1 has been in care for 3 months.
The emergency evacuation procedures shall be practiced monthly with all caregivers and children in care during all shifts that children are in care.
The family day home did not practice a emergency evacuation procedure for the month of November.
