Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on July 3, 2025, as a part of the licensure period. The on-site inspection began at 10:30am and ended at 11:30am. The inspector reviewed compliance in the areas listed above. There were six children present and two caregivers; the point total was 14. The inspector reviewed six children?s records and four career/household member(s) records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
1Records for children #1 and #6 did not have the parent's signature for the annual review of the emergency contact information. Child #1 and Child #6 have been in care over a year. The annual updates expired one month ago.
