The license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises.

The current violation notice and inspection summary for the last inspection dated 07/03/2024 was not posted. The 12/09/2024 inspection summary and violation notice was posted.
Plan of Correction: Moving forward, the provider will print and post the most current inspection summary and violation page.
Standard 8VAC20-800-180-A
The provider shall obtain for each caregiver and adult household member a current Report of Tuberculosis(TB) Screening form every two years from the date of the first screening.

Caregiver #1 did not have documentation of a current TB screening. Documentation of a TB screening was dated 10/18/2023 and had been expired for almost two months.

Plan of Correction: Caregiver #1 and Household Member #1 will get a TB screening completed today. Moving forward, the provider will ensure to keep up with expiration dates for TB screenings for all caregivers and household members.

Caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

Caregiver #2 did not have any documentation of sixteen clock hours of annual training.
Plan of Correction: Caregiver #2 will complete the annual sixteen hours of annual training.
Standard 8VAC20-800-90-C
The family day home shall obtain documentation of additional immunizations for a child who is not exempt from the immunization requirements once every six months for children under the age of two years.

Child #3, 17 months old, did not have documentation of additional immunizations in the record. Documentation of the last immunization was when Child #3 was seven months old.
Plan of Correction: The provider will contact the parents of Child #3 and obtain the most current immunization record. Moving forward, the provider will ensure to get a copy of the updated immunization records for children under the age of two once every six months.
