Inspector Notes

An unannounced monitoring inspection was conducted today between 8:42am and 9:50am. There were 8 children present with 2 caregivers supervising for a total of 19 points within the supervision guidelines. The children were observed playing with trucks, story time, music and movement, and playing with other age-appropriate toys. Positive interactions were observed during this inspection. The inspector reviewed compliance in the areas administration, physical plant, staffing and supervision, programing, fire drill log, menu, attendance and emergency procedures. A total of 7 child records and 2 caregiver?s records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the programs.

Based on record review, it was determined the provider did not review with the parent the emergency contact information to ensure the information is correct and obtain the parent's signed acknowledgment of the review.
Evidence:
1) Child #6 Date of enrollment was 8/16/21 and there is no annual review documented.
2) Child #7 Date of enrollment was 10/4/21 and there is no annual review documented.
Plan of Correction: I will have the parents review the emergency contact information today.

Based on record review, it was determined documentation of additional immunizations was not obtained every six (6) months for children under the age of two (2) years.
Evidence:
1) Child #1 (DOB 11/9/21) record did not have additional immunizations. Last update was 7/11/22.
2) Child #2 (DOB 8/2/21) record did not have additional immunizations. Last update was 2/11/22.
3) Child #3 (DOB 12/5/21) record did not have additional immunizations. Last update was 3/11/22.
4) Child #6 (DOB 2/23/20) record did not have additional immunizations. Last update was 7/31/21.
5) Child #7 (DOB 1/27/20) record did not have additional immunizations. Last update was 9/27/21.
Plan of Correction: I will tell the parents I need an updated immunization record by 2/10/23.
