Inspector Notes
An unannounced renewal inspection was completed on 08/22/2024. The on-site inspection began at 10:45 a.m. and ended at 1:24 p.m. The inspector reviewed compliance in the areas listed above. There were 28 children present and 6 staff. The inspector reviewed 5 children?s records and 6 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 09/04/2024. A POC submitted after this date will not appear on the public website.
Violations
4Based on record review, the provider failed to ensure that staff underwent criminal
background checks every 5 years.
Evidence:
The record for S, contained national criminal background checks dated 02/18/2018 and
01/18/2024.
The record for S3, contained a national criminal background check dated 01/22/2019 and 04/18/2024.
The record for S6, contained a national criminal background check dated 01/25/2019 and 04/19/2024.
Screening form every two years from the date of the previous tuberculosis screening.
Evidence:
The TB screenings for S3 and S6 were dated 06/07/2021 and 08/07/2024.
Department of Education-sponsored orientation course within 90 days of employment.
Evidence:
The record for S2, employed 03/16/2023 contained a certificate of completion of the
Virginia Preservice Training dated 09/11/2023.
Based on staff record review and observation, the provider failed to ensure that each
classroom where children are present shall have current certification in cardiopulmonary
resuscitation (CPR) and first aid.
Evidence:
The record for S4 did not contain evidence of current CPR and first aid training.
S4 was observed in a classroom alone with six children.
