Inspector Notes

An unannounced monitoring inspection was conducted on 2/3/2023 from 8:30 am-10:00 am. There were 10 children (24 points) supervised by 3 direct caregivers. The home, 3 staff records, 5 children?s records, evacuation drills, and policies were inspected. Children were observed engaging freely and eating breakfast. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Based on record review, the provider did obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Evidence: The record of child #1 did not obtain a physical examination prior to child's attendance or within 30 days after the first day of attendance. Child #1 enrollment date is 6/22/2021.
Plan of Correction: Parent will provide to me today and I will put in file.
Standard 8VAC20-800-180-A
Based on record review, the family day home did not obtain from each caregiver and adult household member a current Report of Tuberculosis Screening form, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.

Evidence: The most recent previous tuberculosis screening for caregiver #1 was dated 10/12/2020. The tuberculosis screening expired on October 12, 2022.
