Inspector Notes

An unannounced non-mandated focused inspection was initiated on 07/14/2022 from 3:05 pm to 3:40 pm. The inspector reviewed compliance in the areas of administration, staff qualifications and training, and staffing and supervision. The purpose of the inspection was to also investigate alleged self-report incident that occurred on June 17, 2022 regarding a forbidden action against a child. One staff record was reviewed, three staff members were interviewed, and attendance documentation was reviewed.
There were no violations based from the self-report, however, information gathered during the focused inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation and interview, the center failed to ensure the care and protection of children at all times.

Evidence:
1. In Classroom #1, there were eight children, out of sight and sound supervision. When the Licensing Inspector entered the room there was not a staff member present in the classroom. The staff member was outside of the classroom in the restroom assisting two children.
2. In Classroom #2, there was one child left out of sight and sound supervision. When the Licensing Inspector asked Staff A after observation of the classroom, "You have nine children?" Staff A replied "No, I have ten. One is sitting near the fireplace because she is sick." Licensing Inspector then observed child sitting in hallway near the fireplace.

Evidence:
1. The most current inspection posted was 11/15/2021.
2. The last inspection conducted was on 04/28/2022.
Plan of Correction: I didn't realize that I needed to hang up the inspections if there were not any violations. I will hang the inspection up.
