Inspector Notes
An unannounced monitoring inspection was completed on 08/23/2022, to determine if the licensee has complied with the requirements of the Special Order issued by the Superintendent on 05/11/2022. The inspector was on site from 9:03 am to 9:50 am. There were 70 children present with 12 staff supervising.
The Special Order, issued under the authority of ? 22.1-289.023 of the Code of Virginia, requires that the licensee notify in writing the parents, guardians or other responsible persons (families) of the children currently in care regarding the health and safety violations listed in the Notice of Intent. Within 15 days of the completion of the required written notification, the licensee shall supply evidence to the assigned Licensing Inspector that the notifications have been made. The licensee shall post (i) any Violation Notices attached to the Notice of Intent, (ii) the Notice of Intent dated April 15, 2022 and (iii) the Special Order dated May 11, 2022, in a prominent place at each public entrance of the premises of Willow Tree Academy, LLC, 2359 Jefferson Highway, Waynesboro, Virginia 22980 for a period of twelve (12) months
