Inspector Notes
The licensing inspector initiated an unannounced inspection on 01/05/2024 and concluded on 02/06/2024 in response to a complaint received by the department on 12/27/23 for allegations regarding special feeding needs and supervision. Interviews, record reviews and policy reviews were conducted relating to the allegations. The information gathered during the investigation support the allegation, so the complaint is determined to be valid, and violations were documented on the violation notice issued to the facility.

Violations
2Evidence:
1. Per interview with adult 1: On 12/27/23 at about 9:45 AM, staff 3 was alone with four infants, one of which was crying while face down on a Boppy pillow on the floor; when asked if she was going to move child 2, staff 3 replied ?Not now."; Adult 1 walked to the office and then walked back to the classroom with staff 4 and staff 5 to find Child 2 was still face down in the Boppy pillow, now asleep. Adult 1 reported about 15 minutes passed between the time child 2 was first observed face down on the Boppy pillow to when they returned to the classroom to find child 2 sleeping face down on the Boppy pillow.
2. Interview with staff 4 confirmed that upon arrival to the classroom, staff 4 witnessed child 2, 5 months old, sleeping on the Boppy pillow face down.
3. The warning label on the Boppy pillow states the following:
-Never allow baby to sleep on the Boppy pillow.
-Do not allow baby to lie face down on a Boppy pillow.
Evidence:
1. Staff 6 stated child 1 would normally take a bottle every 2 ? to 3 hours. Child 1 would drink two to three ounces out of each bottle.
2. Documentation and interviews indicate that child 1, age 7 months, was in care of the center from approximately 9:30 am ? 4:30 pm on 12/27/2023 and the only liquid the infant was provided during that time was two ounces of breastmilk.
3. Staff 2 stated she was not sure why they did not give child 1 more bottles.
