Inspector Notes

An unannounced inspection was initiated on 07/22/2024 and completed on 07/30/2024 in response to a complaint received by the licensing office on 07/11/2024 relating to allegations of inappropriate behavioral guidance and mandated reporting. The inspector reviewed one staff record. This inspection included interviews and observations.
The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 08/28/2024 @ 5:00 PM. A POC submitted after this date will not appear on the public website.

Based on interview, a center staff failed to report suspected child abuse or neglect of a child in care immediately to the local Department of Social Services (DSS).

Evidence:

Staff 1 witnessed staff 2 shove a three year old child to the ground and failed to report the incident to the local DSS.
Plan of Correction: The administration has talked to Staff 2 about age-appropriate discipline and age-appropriate techniques for redirection for a positive change.
Standard 8VAC20-780-400-B
Based on interviews, the center failed to ensure behavior guidance is constructive in nature, age and state appropriate, and is intended to redirect children to appropriate behavior and resolve conflicts.

Evidence:

The licensee confirmed that Staff 2 was observed getting in children's faces and yelling at them since May.
Staff 2 has been observed getting in children's faces and yelling at them multiple times since May.
Plan of Correction: The center has been working with staff 2 on classroom techniques and best practice with the preschool classroom. Staff 2 also had an initial coaching visit on 07/10/2024. However, in observation we decided that Staff 2 would best be fit for an older age group of children. We have since moved Staff 2 to the Pre-K program, as a co-teacher and Staff 2 has blossomed. Staff 2's demeanor is much more positive, and goals have been set to everyday activities where they are the focus and are being accomplished in classroom.

Based on observation and interview, the center failed to post a copy of the special order issued by the department in a prominent place at each public entrance of the licensed premises to advise consumers of serious or persistent violations.

Evidence:

The Special Order signed on 05/16/2024 was not posted in the center, as required.
