During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site focused inspection was initiated on 10/22/2024 and completed on 10/22/2024. The on-site inspection, conducted in conjunction with another Licensing Inspector, began at 8:13 AM and ended at 10:00 AM. The inspector reviewed compliance in the areas listed above. There were 64 children present and 15 staff. The inspector reviewed one child?s record and one staff's record on-site. This inspection included document review, observations, interviews, and tour of the facility. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/04/2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-280-B
REPEAT VIOLATION, SYSTEMIC VIOLATION Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.
A spray can of Febreze air freshener with warning label, ?Keep out of reach of children,? were unlocked on top of the paper towel holder in the hall bath used by children.
Gallons of paint, Zep glass cleaner, Gorilla Contact Adhesive and other hazardous substances were on the floor and shelves in the hall closet. The closet was unlocked.
Plan of Correction: Febreze placed in locked cabinet in classroom.
Janitor's closet; We purchased a 2nd lock.

In areas used by preschool age or younger children, electrical outlets shall have protective covers. An outlet in the Toddler room did not have a protective cover. The youngest child in care was 15 months.
Plan of Correction: Teacher given checklist to ensure classroom safety.

REPEAT VIOLATION, SYSTEMIC VIOLATION The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential. The posted list in the toddler room did not include one child's dietary restriction.
Plan of Correction: Allergy posted after notice that the child's information was missing.

Combs, toothbrushes, or other personal articles shall be individually assigned.
A child's pacifier was not individually assigned.
Plan of Correction: Teacher spoken to with a reminder of all children's belongings to be labeled.
Standard 8VAC20-780-440-B
Cribs, cots, rest mats, and beds shall be identified for use by a specific child.
In the infant room one crib was not labeled for a specific child and the crib was in use by an infant.
Plan of Correction: Staff were told even when classrooms condensed to ensure all children have a personal sleeping space.
Standard 8VAC20-780-500-A
Children?s hands shall be washed with soap and running water after toileting, or any contact with blood, feces or urine. Staff changed a child?s diaper in the toddler room and did not wash the child's hands with soap and water afterwards.
Plan of Correction: Staff was reprimanded and given an additional hygiene training to complete.

Tables shall be sanitized after each use for feeding.
Children were observed playing at the tables after breakfast. The tables had spilled milk on them.
Plan of Correction: Teacher was reprimanded. Both teachers were given sanitation training to ensure standards are met.
