Inspector Notes
An unannounced inspection was initiated on May 28, 2025 and completed on May 28, 2025 in response to a complaint received by the licensing office on May 21, 2025 relating to allegations of supervision, ratios and confidentiality. The inspector reviewed two staff records on-site. This inspection included interviews, record review, tour of the facility, and observations. The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
