Inspector Notes
An unannounced, on-site focused inspection was initiated on July 16, 2025 and completed on July 16, 2025 as a part of a Consent Agreement. The Department entered into a Consent Agreement with the facility on April 16, 2025. The on-site inspection began at 10:25 a.m. and ended at 12:00 p.m. The inspector reviewed compliance in the areas listed above. There were 75 children present and 15 staff. The inspector reviewed two staff records on-site. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
