During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Inspector Notes
An unannounced, on-site monitoring and focused inspection was initiated on October 1, 2025 and completed on October 1, 2025 as part of a Consent Agreement. The Department entered into a Consent Agreement with the facility on April 16, 2025. The on-site inspection began at 9:45 a.m. and ended at 1:10 p.m. The inspector reviewed compliance in the areas listed above. There were 68 children present and s14staff. The inspector reviewed six children?s records and six staff records on-site. The inspection included record review, tour of the facility, observations, and interviews. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
