Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including phone contacts, documents review, and interviews.
A subsidy health and safety (SHSI) inspection was initiated on 08/08/21 and concluded on 08/19/21. There were 103 children present with staff maintaining ratios. A licensing inspection was also conduced o this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

The center failed to have information on a food allergy and actions to be taken in an emergency situation and information on other physical problems.
Evidence:
1. Child record #6 did not contain updated allergy information or an allergy plan.
Plan of Correction: Director will get from the parent updated medical information from the parent and an allergy plan will be completed. All staff will be made aware of the allergies and the plan. MAT training will be investigated.
