An unannounced monitoring inspection was initiated on and completed on 5/15/2025 in response to a self-report received by the licensing office on 5/8/2025 relating to Staffing and Supervision. The inspector reviewed five children?s records and one staff record on-site. This inspection included document review, tour of the facility and interviews.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
