Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated and concluded on 04/23/2021. The Provider was contacted by telephone to initiate the inspection. There were (4) children present, (9) points, with (1) caregiver. The inspector emailed the Provider a list of items required to complete the inspection. The Inspector reviewed (2) children?s records, (1) caregiver record, and (2) household member records submitted by the Family Day Home (FDH) to ensure documentation was complete.

Information gathered during the inspection determined (2) non-compliances with applicable standards or law and were documented on a Violation Notice issued to the FDH.

Standard 22VAC40-111-180-A
Based on review, 1 of 3 staff and household member files indicated that the Provider did not obtain for each adult household member a current report of Tuberculosis (TB) documentation every (2) years.

Evidence -
1. On the date of inspection, TB documentation on file for Household Member #2 was dated - 10/12/2018.
2. The documentation on file would have expired after 10/12/2020. No updated TB documentation was available for review on the date of inspection.
Plan of Correction: Updated TB documentation shall be obtained for Household Member #2 and placed on file.
Standard 63.2-1721.1-A
Based on review, 3 of 3 staff and household member files indicated that the family day home (FDH) did not undergo a background check (in accordance with subsection B prior to issuance of a license and) every five years (thereafter).

Evidence -
1. On the date of inspection (04/23/2021), the following individuals had an expired search of the central registry from Virginia (CPS check) on file: Staff #1 (finding date - 12/07/15), Household Member #1 (finding date - 12/07/15), and Household Member #2 (finding date - 11/09/15).
2. Each CPS check expired (5) years after the finding date.
Plan of Correction: A new request of the central registry shall be obtain for all three and placed on file.
