Inspector Notes

A monitoring inspection was conducted on 04/18/2024. There were a total of 1 child (2 points) observed with 1 caregivers present. The children were engaged in the following: playing with toys. The inspector reviewed compliance in the areas of administration, personnel and household members, physical environment, care of children, handwashing, diapering, medication, emergencies and nutrition. A total of 1 child records and 5 caregiver/household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of caregiver and household records, the provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form every two years from the date of the first screening.
Evidence:
Caregiver 1 had a TB test that expired 03/07/2024.
Household Member 1 had a TB test that expired 03/14/2024.
Household Member 2 had a TB test that expired on 04/07/2024.
Plan of Correction: We will schedule the tests and xray if needed as soon as possible.
