Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 9/4/2024 and completed on 9/4/2024. The on-site inspection began at 9:30AM and ended at 10:20AM. The inspector reviewed compliance in the areas listed above. There were 3 children present and 1 caregiver; the point total was 7. The inspector reviewed 3 children?s records and caregiver/household member records. This inspection included document review, a tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Based on record review, a household member did not have a follow up Sworn Disclosure Statement five years after the previous statement. Evidence: Household member A's Sworn Disclosure Statement expired 8/20/2024.
Plan of Correction: Household Member A signed a new Sworn Disclosure Statement.
Standard 8VAC20-800-690-D
Based on observation, the diapering surface was not nonabsorbent and washable. Evidence: There was a tear in the changing table pad, exposing the nonabsorbent material.
Plan of Correction: A new changing pad was purchased and replaced the torn pad.
