Inspector Notes

An unannounced mandated inspection was completed on 04/01/24 from10:30am until 11:45. There were 8 children (a total of 14 points) and two providers present. Five children?s records and two provider records were reviewed.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition.

Information gathered during the inspection determined non-compliance with applicable standards or law and violation (s) were documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
Based on record review, the provider failed to get physical exams within 30 days of first day or attendance.
Evidence:
Child #1 (DOE 6/20/23) and Child #4 (DOE 8/25/23) did not have physical exams in the file.
Plan of Correction: The provider will get the physicals from the guardian.
Standard 8VAC20-800-180-A
Based on record review, the provider did not have repeat TB screenings for each caregiver.
Evidence:
TB screening for provider #1 and #2 expired 3/2024.
Plan of Correction: Both providers will get their T screenings completed.
Standard 8VAC20-800-460-C
Based on record review, pets did have up to date vaccinations.
Evidence:
1. Pet #1's rabies vaccination expired 1/25/24. Pet #2's exp. 1/125/24.
Plan of Correction: Pets will get their updated vaccinations. Pets are not allowed around the children.
