During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced on-site inspection was initiated and completed on 8/7/25. The on-site inspection began at 3:00 and ended at 4:30. The inspector reviewed compliance in the areas listed above. There were 5 children for a total of 9 points were present with two providers present. The inspector reviewed 5 children?s records and 2 provider records at the main site. This inspection included document review, tour of the facility, interviews, and observation.

Information gathered during the inspection determined non-compliance with the applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-60-A
Providers are require to maintain an up to date record at the home for each enrolled child.

Child records #1-#4 had not been updated in over 1 year. The files had been last updated 3/2024.
Plan of Correction: The provider will update all the children's records.
Standard 8VAC20-800-90-C
The family day home shall obtain documentation of additional immunizations for children once every 6 months for children under the age of two years.

Child #5's immunizations was not up to date; child #5 had been in care for at least 6 months.
Plan of Correction: Parents will be asked for the updated records.
