Inspector Notes
An unannounced monitoring inspection was conducted on 8/26/22 from1:00pm t0 1:40pm. There were no children in care. A walk through of the family day home was completed. 1 caregiver and 3 household member's records were reviewed. First aid kit was complete. There were no significant changes in the physical space or children's equipment present.

Based on staff interview, the family day home did not obtain results of the central registry findings every 5 years for adult household members.
Evidence:
1. The central registry finding results for the provider, Household Member # 1 and Household Member # 2 expired on 3/9/21.
2. Household Member # 2's central registry findings expired on 2/8/21.
3. The provider stated that she had obtained updated results in April 2021 but could not find them.
