Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 7/11/2024 and completed on 7/11/2024. The on-site inspection began at 10:30am and ended at 11:45am. The inspector reviewed compliance in the areas listed above. There were 11 children present (37 points) and 3 staff. The inspector reviewed 5 children?s records and 4 staff records on-site on 7/11/2024. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on review of staff records, the provider did not obtain a search of the central registry for their staff.

Based on review of children's records, the provider did not obtain documentation of additional immunizations once every 6 months for children under the age of two years.
