Inspector Notes

An investigation was conducted jointly with local authorities in response to a self-reported incident at the family day home.

Based on in person inspection, multiple adult interviews, and a review of video recordings, the preponderance of evidence does not support any violations regarding physical punishment.

Other violation cited was found during the course of the investigation and was not part of the original report.

Standard 8VAC20-800-30-A
Based on the provider's statement, the facility has not operated in compliance with the standards and the terms of the license, and with relevant federal, state or local laws and other relevant regulations.
Evidence:
1. The Provider stated she was providing care, outside of the hours of operation as stated on the most recent application, on the evening of Friday 11/04/2022 when an incident occurred with the parent of a child in care. Hours of operation are listed as 8:00AM-5:00PM.
2. The Provider stated she does provider care occasionally for parents, outside of normal operating hours. The Zoning permit issued by Fairfax County requires that the operating hours for the facility shall be limited to 5:00a.m. to 7:00 p.m. Monday through Friday.
Plan of Correction: I will contact the Zoning to see about amending the hours for special occasions.
In the mean time I will not provide care outside of the operating hours.
