Inspector Notes
An unannounced monitoring inspection was conducted today. There were 12 children (33 points) with 3 caregivers present.
Children were observed washing hands, toileting, playing with a variety of toys and games and eating.
Emergency evacuation drills, first aid supplies, emergency supplies, menu, bathroom supplies, rabies vaccination and the refrigerator temperature were all in compliance.
4 caregiver and 4 child records were reviewed. See Violation Notice for non-compliances cited.

Evidence:
1 of 4 records reviewed was not up-to-date. The record was signed by the parent 4/4/21, the form in use states it is to be updated annually. (Child 2).
1 of 4 records reviewed was dated 12/14/21; however, the child's date of birth is 06/10/22 (Child 1).
Evidence:
The freezer temperature was observed to be 8 degrees on the freezer thermometer.
