Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on February 8, 2022 and concluded on February 11, 2022. The director was contacted by telephone and a virtual inspection was conducted. There were 12 children present, ranging in ages from 4 months to age 2, with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of 2 staff records and staff interviews, the licensee did not ensure tuberculosis (TB) documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

Evidence: The record for staff #2 did not include TB screening dated within 30 day of employment. TB screening was dated 6-17-2021 and date of employment 1-18-2022.
Plan of Correction: Staff member will achieve a up to date TB screening.
