An unannounced monitoring inspection was initiated on 11-7-2024 and completed on 11-25-2024, in response to a self-report received by the licensing office on 10-29-2024 relating to relating to an injury requiring outside medical attention and areas of Administration and Programs. The inspector reviewed 2 children?s records. This inspection included document review, tour of the facility, interviews and observations.
