Inspector Notes
An unannounced inspection was initiated on 11/7/2024 and completed on 12/11/2024 in response to a complaint received by the licensing office on 10/31/2024 relating to allegations of Physical Plant, Staffing & Supervision,Special Care Provisions & Emergencies and Special Services. This inspection included document review, tour of the facility, interviews, observations and measurements.
The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Though not a part of the original complaint, violations that were not reported were found related to Staffing & Supervision and Special Care Provisions & Emergencies are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/18/2024. A POC submitted after this date will not appear on the public website
Standard 8VAC20-780-330-B
Playground equipment with moving parts or climbing apparatus must have appropriate fall zones that encompass sufficient area to include the child's trajectory in the event of a fall. Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus. The resilient surfacing for merry-go-cycle measured 1 ? inches or less and a minimum of 6 inches is required.

Standard 8VAC20-780-340-F
Children under 10 years of age shall always be within actual sight and sound supervision of staff. On11/7/2024,3 infants were observed left unsupervised when Staff #1, in the infant room went into the restroom to change another child?s diaper.
Plan of Correction: There were 2 staff members on staff with 4 children in care. One staff member walked away from the class, then 2nd staff member began to do diaper changes, spoke with staff about starting diaper changes when the assistant is not in the room.

(Repeat Violation)
Staff hands and children?s hands shall be washed with soap and running water after toileting, or any contact with blood, feces or urine. On 11/21/2024 Staff # 2 changed a child?s diaper and did not wash their hands or the child's hands with soap and water.
Plan of Correction: Coached and developed with staff member the importance of washing children's hands after diapering. Audited Staff 11-21-2024
Standard 8VAC20-780-500-B
The diapering surface shall be a nonabsorbent surface for diapering or changing and shall be cleaned with soap and at least room temperature water and sanitized after each use. On 11/21/2024 the diaper pad in the toddler room had a tear (less than 1/2 inch) and therefore it was no longer nonabsorbent. Staff #2 was observed changing a diaper in the toddler room and cleaned the diaper changing surface but did not sanitize the surface.
Plan of Correction: Diaper Changing pad was replaced. Diaper changing pad was sanitized with wrong solution. Coached a& developed w/ staff.
Standard 8VAC20-780-560-F
When centers choose to provide meals or snacks, they shall follow the most recent, age-appropriate nutritional requirements of the Child and Adult Care Food Program of the United States Department of Agriculture (USDA). USDA indicates children ages 1-2 shall have 4 fluid ounces of milk for lunch. On 11/21/2024 the center served during lunch approximately 2 fluid ounces to the children in the toddler room (ages 16 months to age 2).
Plan of Correction: This age of children are transitioning from sippy cups, their milk is given to them in part for spillage purposes. For corrective measure , an to ensure proper amounts given sippy cups were re-introduced.
