Inspector Notes

Licensing Inspector conducted an unannounced Religiously Exempt Code Monitor Inspection on October 4, 2023. There were 9 children ages 2 1/2 to 4 years old present under the direct care of 2 staff. The Licensing Inspector reviewed 4 children?s records and 2 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the Center failed to obtain documentation of proof of identity and age and previous child day care and schools for each child enrolled.

EVIDENCE:
Record for Child #1 did not include documentation that the Center confirmed proof of age and identity (birth certificate).
Plan of Correction: Center Director will request documentation of proof of age/identity from the child's parents.

Based on inspection & discussion with the Center Director, the Center failed to have a carbon monoxide detector in each building that was built before 2015 and that houses a child day program serving preschool-age children.

EVIDENCE:
A carbon monoxide detector was unavailable.
Plan of Correction: Center Director will purchase and install a carbon monoxide detector.
