Inspector Notes

A monitoring inspection was conducted on February 23, 2022, following all COVID protocols. There was 1child present, who is 3 years old, with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 household member/staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
Based on review of two children's records, the provider did not obtain documentation of a physical examination prior to child #1 first day of attendance or within 30 days after the first day of attendance.
Evidence
There is no documentation of a physical examination for child #1 prior to their attendance or within 30 days after the first day of attendance.
Plan of Correction: i will get the physical and put it in their record.
Standard 8VAC20-800-90-A
Based on review of two children's records, the provider did not obtain current documentation that the child has been adequately immunized according to the Code of Virginia and applicable State Board of Health regulations for child #1.
Evidence
There is no documentation of a current immunization record for child #1 prior to their attendance.
Plan of Correction: i will get a current immunization record and put it in their record.
