During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on December 10, 2025 and completed on December 10, 2025, as a part of the current licensure period. The on-site inspection began at 10:30am and ended at 11:30am. The inspector reviewed compliance in the areas listed above. There were 5 children present and 1 caregivers; the point total was 15. The inspector reviewed 3 child files and 2 caregiver/household member files on-site. This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Children's records are required to contain documentation of the parent's signed acknowledgment of review of the child's emergency contact information.

Child C did not have documentation of the annual review since enrollment. This is overdue by 2 months.
Plan of Correction: Parent is out of town and the other parent does not speak much English, so I will have them sign it as soon as they are back in the country.
