Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 5/2/2025, as a part of the licensure period. The on-site inspection began at3:50 PM and ended at 5:30 PM. The inspector reviewed compliance in the areas listed above. There were 53 children present and 7 staff. The inspector reviewed 5 children?s records and 3 staff records on-site and 3 staff records electronically on 5/7/2025. This inspection included:

? observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations

Standard 8VAC20-780-160-A
Each staff member shall submit documentation of a negative tuberculosis screening that was completed within the last 30 calendar days of the date of employment. The tuberculosis screenings for two of the staff were obtained after the date of employment.
Plan of Correction: Provider response:
In the future, staff will be required to obtain tuberculosis screenings within 30 days prior to employment.
