During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 03/19/2026. The on-site inspection began at 10 am and ended at 11:30 am. The inspector reviewed compliance in the areas listed above. There were (7) children present and (2) caregivers; the point total was (13). The inspector reviewed (7) children?s records, (2) caregiver records, and (3) household members records on-site on 03/19/206.

This inspection included: document review (i.e. Files, injury logs (none), policies and procedures, and emergency drill logs), tour of the facility, interviews, and observations.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI.

Information gathered during the inspection determined (2) non-compliance with applicable standards. These violations were documented on the violation notice issued to the home.

Before the child's first day of attendance, the provider shall obtain the parent's written acknowledgement of the receipt of the information regarding family day home operations and administration.

There were no parent acknowledgement available for Child #4 (who was provided care in February and March 2026. The FDH's Medication Administration ? Decision to Administer form was not available for Child #4.
Plan of Correction: A signed Medication Administration ? Decision to Administer from the parent's of Child #4 will be obtained and placed on file.

Annually, the provider shall review with the parent the emergency contact information required in each child?s record to ensure the information is correct and shall obtain the parents signed acknowledgment of the review.

Records for children #1, and #7 did not have a parent?s signature for an annual review of the emergency contact information. Child #1, and #7 last documented reviews were from over (13) thirteen months ago.
Plan of Correction: An annual review of the emergency information for both Child #1, and Child #7 will be conducted and a signature & date will be obtained as verification. This verification will be placed in each child's file.
