Inspector Notes

An unannounced monitoring inspection was conducted on April 3, 2023, from 10:30 a.m. to 12:30 p.m. There were 55 children present, ranging in ages from four months to five years old with eight staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and eight staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-160-A
REPEAT VIOLATION
Based on review of records, observation and interview, the center failed to ensure that each staff member submitted a negative tuberculosis (TB) screening at the time of employment and prior to coming in contact with children.
Evidence:
1. Staff #8?s start date on file was 3/27/2023. Staff #8 was observed in a classroom with children.
2. Staff #8 did not have TB screening on file. Staff #1 confirmed that #8 did not submit a negative TB screening at the time of employment.

Based on observation, the center failed to ensure that over-the-counter skin procedures were not kept beyond the expiration date.
Evidence:
1. In the toddler classroom, there was Destin diaper cream labelled with Child #6?s name. The expiration date was 9/2020.

REPEAT VIOLATION
Based on interview and observation, the center failed to ensure that when food is brought from home, it is clearly dated and labeled in a way that identifies the owner.
Evidence:
1. In the toddler classroom, there was one lunchbox without a name or date. Staff #7 confirmed that the lunchbox was not labeled with a name or date.
2. In the 3?s and 4?s classroom, there was two lunchboxes without a name or date. Staff #4 and Staff #3 confirmed that the lunchboxes were not labeled with a name or date.
