Inspector Notes
An unannounced monitoring inspection was conducted on 06/07/2024 from 11:15 a.m. to 3:10 p.m. under the supervision of the Licensing Administrator. There were 35 children present, ranging in ages from six months to ten years old with eight staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations
6Evidence:
1. The carpet in the 4?s classroom had multiple snags and loose strings, two of which were large enough to cause a tripping hazard.
Evidence:
1. The mulch on Playground A measured five inches in various places.
2. The mulch on Playground B measured between two and 4.75 inches in various places
3. The mulch did not meet the minimum safety standard of six inches.
Evidence:
1.Upon request for the injury prevention plan, the Program Director could not locate the injury prevention plan, nor the date that was last updated.
Evidence:
1.The allergy lists in the school age room, the 3?s room, the 2?s room, the infant room and the 4?s room were not dated. Staff #5 confirmed that the lists were not dated.
2.Child #3 had acai berry listed as an allergy in the record. Interviews with PD and licensee revealed there is no allergy, but an intolerance and parental preference which was not listed on the list of children?s allergies, sensitivities, and dietary restrictions in any of the classrooms.
Evidence:
1. In the infant classroom, Staff #3 was observed to be the only staff in the classroom working with the children in care. There was no evidence on file that Staff #3 had current certification in CPR. Staff #3 confirmed that they did not have current certification in CPR.
2. In the 4?s classroom, Staff #1 was observed to be the only staff in the classrooworking with the children in care. There was no evidence on file that Staff #1 had current certification in CPR. Staff #1 confirmed that they did not have current certification in CPR.
Evidence:
1. In the infant classroom, Staff #3 was observed to be the only staff in the classroom working with the children in care. There was no evidence on file that Staff #3 had current certification in first aid. Staff #3 confirmed that they did not have current certification in first aid.
2. In the 4?s classroom, Staff #1 was observed to be the only staff in the classroom working with the children in care. There was no evidence on file that Staff #1 had current certification in first aid.
