Inspector Notes
A monitoring/renewal/complaint inspection was conducted on 4/21/2022. There were 18 children present, ranging in ages from 2 yrs to 5 yrs, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
4Evidence:
The record for staff 3 did not include documentation of any annual training regarding the prevention of emergencies due to allergies. Staff 2 had an open can of cashew nuts on the staff desk in the school age classroom when the facility has a nut free p[olicy due to a child's allergy.
Appropriate action will be taken with the staff person to ensure detailed training addressing the facilities policies regarding prevention of children's allergies and the nut free status of the facility.
Evidence:
1. The blankets to be used by the children during naptime were observed to be piled once atop another on a low table in the toddler classroom allowing for potential cross contamination.
2. Staff 2 acknowledged that the blankets were piled one atop another in the toddler classroom prior to lunch time.
Staff were immediately instructed to remove the blankets and place in each's child's cubby. All staff will be reminded of the importance of keeping children's individual articles separate to avoid cross contamination.
Evidence:
1.There is an albuterol inhaler for child 4. The prescription label indicated that the medication was to be discarded after December 2021.
2. Staff 2 verified that the prescription label indicated that the medication was to have been discarded after December 2021.
The albuterol medication did not expire until April 2022. Staff did not see the conflicting instructions on the prescription label. All staff certified to administer medication will be trained to read all instruction on prescriptions very carefully and review any questions with the parent/physician.
Evidence:
1. Staff 2 verified that the gold colored van is the vehicle currently being used to transport school aged children from school. The state inspection sticker expired in March 2022.
2. Staff 1 verified that the inspection sticker expired in March 2022.
The van was immediately scheduled for an inspection. Proof of passing inspection was submitted to the inspector on 04/22, 2022.
