Inspector Notes

An unannounced renewal inspection was conducted on December 12, 2022. There were 4 children (15 points) with 2 caregivers supervising. The home, children's records, caregiver records, household member records, menu's, first aid kit, attendance log and evacuation logs were reviewed. The children were observed napping and playing with floor toys. Areas of non-compliance were cited on the violation notice. Based on a review of children's records, the provider failed to obtain all required documentation for each child enrolled.

Evidence:
Child #1, #2,#3 and #4's records do not contain documentation of the names of persons other than the custodial parents who are authorized to pick up the child.
Chile #3 and #4's records do not contain documentation of the telephone number of each custodial parent's place of employment.
Plan of Correction: I will get the information from parents and place in children's files.
