Inspector Notes
A monitoring inspection was initiated on 06/21/2023 and concluded on 06/21/2023. There were 5 children present, ranging in ages from 21 months to 11 years, with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed/updated.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

1. The record of Staff 1 (DOH 07/2016) contained a criminal history record background check that was dated 04/05/20218, which expired 04/05/2023. Staff 1 has been employed at the center for more than five years.
2. The record of Staff 2 (DOH 07/2016) contained a criminal history record background check that was dated 04/05/2018, which expired 04/05/2023. Staff 2 has been employed at the center for more than five years.

1. There were two lunchboxes in the Toddler Room that were not dated or labeled with a name.
