Inspector Notes

A monitoring inspection was initiated on 06/21/2023 and concluded on 06/21/2023. There were 5 children present, ranging in ages from 21 months to 11 years, with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed/updated.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on record review, the center failed to obtain criminal history record check results every five years for all employees.

Evidence:
1. The record of Staff 1 (DOH 07/2016) contained a criminal history record background check that was dated 04/05/20218, which expired 04/05/2023. Staff 1 has been employed at the center for more than five years.
2. The record of Staff 2 (DOH 07/2016) contained a criminal history record background check that was dated 04/05/2018, which expired 04/05/2023. Staff 2 has been employed at the center for more than five years.
Plan of Correction: I was confused and sent the wrong background check in. I will make an appointment for the fingerprint background checks immediately.

Based on observation and interview, the center failed to ensure that a food from home container is clearly dated and labeled in a way that identifies the owner.

Evidence:
1. There were two lunchboxes in the Toddler Room that were not dated or labeled with a name.
Plan of Correction: We usually use the dot system but did not do it today. We will begin to use tape to write the names and dates on the lunchboxes.

Based on a review of children's records, the center failed to obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence:
1. The record of Child 5 contains documentation of a diagnosed peanut and egg allergy. The record does not contain documentation of a written allergy care plan completed by a physician.
Plan of Correction: I will call the parent today and have them request it from the dr.
