Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 06/17/2025 and completed on 06/17/2025. The on-site inspection began at 9:15 AM and ended at 10:20 AM. The inspector reviewed compliance in the areas listed above. There were 18 children present and 4 staff. The inspector reviewed 5 children?s records, and 4 staff on-site. This inspection included document review, tour of the facility and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
1The records for Children # 4 and #5, enrolled for one week, was missing the following required components: Chronic physical problems and pertinent developmental information and any special accommodations needed.
