Inspector Notes

An unannounced, on-site monitoring inspection was conducted on 7/22/2024, as a part of licensure period. The on-site inspection began at 10:28am and ended at 11:46am. The inspector reviewed compliance in the areas listed above. There were 9 children present and 3 caregivers; the point total was 20. The inspector reviewed 4 children?s records, 1 staff record and 1 household member record on-site on 7/22/24. This inspection included document review of Injury logs, policies and procedures, emergency drill logs, and authorization forms, tour of the care center, interview, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on record review and interview, the family day home did not obtain a qualifying result on a background check within the past five years for an employee.
Evidence: Staff #1's record date of last FBI clearance dated 2/12/2018.
Plan of Correction: Substitute provider to schedule fingerprinting in the next 7 days.
Standard 22.1-289.036-A
Based on record review, the family day home did not obtain an updated criminal history record for an adult living in the family day home every five years.
Evidence: Household Member #1 date of most recent criminal history report (FBI fingerprints) 2/12/2018
Plan of Correction: Provider to notify household member to schedule in the next 7 days.

Based on documentation review, the provider did not obtain updated tuberculosis (TB) tests or screenings for each household member every two years.

Based on review of four children's records, the family day home did not obtain documentation of additional immunizations for a child who is not exempt from the immunization requirements once every six months for children under the age of two years.
Evidence:
Child #1's date of last immunization 8/28/23 will be 2 years old 8/2025, Child #2's date of last immunization 9/10/22 will be 2 years old 10/2024, Child #3's date of last immunization 4/9/22 will be 2 years old 8/2024.
Plan of Correction: Provider will request updated immunization records from parents. Provider will routinely review files for immunization due dates.
