Inspector Notes
An unannounced, on-site monitoring inspection was conducted on 7/22/2024, as a part of licensure period. The on-site inspection began at 10:28am and ended at 11:46am. The inspector reviewed compliance in the areas listed above. There were 9 children present and 3 caregivers; the point total was 20. The inspector reviewed 4 children?s records, 1 staff record and 1 household member record on-site on 7/22/24. This inspection included document review of Injury logs, policies and procedures, emergency drill logs, and authorization forms, tour of the care center, interview, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on documentation review, the provider did not obtain updated tuberculosis (TB) tests or screenings for each household member every two years.
Evidence: Household member #1's date of last TB screening or test 3/30/21
Based on record review the provider did not maintain an up to date record at the family day home for each enrolled child that contained all required information.
Evidence:
1. Child #2's record did not contain the name, address and telephone number of a second designated persons to contact in case of an emergency if the parent cannot be reached.
Evidence: Provider's radio was AM/FM only.

Based on review of four children's records, the family day home did not obtain documentation of additional immunizations for a child who is not exempt from the immunization requirements once every six months for children under the age of two years.
