During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 12/20/2024, as a part of the licensure period. The on-site inspection began at 9:30am and ended at 10:50am. The inspector reviewed compliance in the areas listed above. There were 6 children present and 3 caregivers; the point total was 19. The inspector reviewed 4 children?s records and 6 caregiver/household members' records on-site. This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-210-A
Caregivers shall obtain a minimum of 16 clock hours of training annually.

None of the caregivers (#1,#2 and #3) obtained any training hours in 2024.
Plan of Correction: Provider and all caregivers to schedule 16 hours of training before end of Jan 2025.
Standard 8VAC20-800-60-B
REPEAT VIOLATION
Children's records are required to contain name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached.

The record for Child #2 was missing the name, address and telephone number of 1 designated person to contact in case of an emergency if the parent cannot be reached.
Plan of Correction: Provider to request information from parent again at pick up today.

REPEAT VIOLATION
Annually, the provider shall review with the parent the emergency contact information required in each child?s record to ensure the information is correct and shall obtain the parents signed acknowledgment of the review.
Records for children #1 and #2 did not have a parent?s signature for an annual review of the emergency contact information. Child #1 has been in care for over a year. Child #2 has been in care for over two years without any annual review.
Plan of Correction: Provider to have parents review and sign acknowledgement of review of their child's information at pick up, today.

REPEAT VIOLATION
Documentation of additional immunizations is required once every six months for children under the age of two years. '

Child #2, 3 years old, and has been in care for 26 months, last documented immunization was over 21 months ago.
Plan of Correction: Provider to request information from parent again today.
