During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 7/2/2025 as a part of the licensure period. The on-site inspection began at 9:40am and ended at 10:30am. The inspector reviewed compliance in the areas listed above. There were 7 children present and 3 caregivers; the point total was 23. The inspector reviewed 4 children?s records and 3 caregiver/household members records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

The provider did not obtain documentation of a physical exam of child #4, who has been in care for 10 months.
Plan of Correction: Provider will request documentation from parent today. Provider will review children's' records for required documentation.
Standard 8VAC20-800-110-B
Immunization reports report shall be signed by a licensed physician, the physician's designee, or an official of a local health department.

Immunization report for child #4 was not signed by a licensed physician, the physician's designee, or an official of a local health department.
Plan of Correction: Provider will request parent obtain and forward required signed document today.
Standard 8VAC20-800-60-B
A child's record shall contain name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached.

Child #2's record did not contain address of two designated persons to contact in case of emergency and parent cannot be reached.
Plan of Correction: Provider will request information from parent today. Provider will review children's forms/records for completion.
Standard 8VAC20-800-70-B
The provider shall obtain the parent's written acknowledgement of the receipt of required information.
Child #2s record did not contain the information requirement for the family day home to notify the parent when the child becomes ill and for the parent to arrange to have the child picked up as soon as possible if so requested by the home and requirement for the parent to inform the family day home within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease,
as defined by the State Board of Health.
Plan of Correction: Provider will review document with required information with parent and have parent sign receipt of information, at pick up today. Provider will review childrens' records for completion.
