The inspection document posted was the acknowledgement form from 2/2/2024 and not from most recent inspection7/2/2025.

Standard 8VAC20-800-210-A
Caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

Caregiver #1 only obtained 1.5 hours of training in the past year.
Caregivers #2, #3 and #4 did not obtain any training hours in the past year.
Plan of Correction: All caregivers will complete required training hours in the next year. Provider will ensure training hours are completed and tracked in a timely manner.

A child's record shall contain names of persons other than the custodial parents who are
authorized to pick up the child.

The record for child#2 did not contain names of persons other than the custodial parents who are authorized to pick up the child.
Plan of Correction: Provider will obtain information from parent today. Provider will routinely review children's records to ensure completion.

The family day home shall obtain documentation of additional immunizations for a child who is not exempt from the immunization requirements once every six months for children under the age of two years.

The record for child #4 (13 months old) did not have documentation of immunizations once every six months. DOL immunization was over six months ago.
Plan of Correction: Provider contacted parent and requested updated immunizations. Provider will routinely review children's records to ensure completion and up to date information is in record.
