Inspector Notes

An unannounced monitoring inspection was conducted on 3/22/2023 from 10:15 am-11:40 am. There were 8 children directly supervised by 2 caregivers. The home, 3 caregiver records, 5 children?s records, 2 household member records, evacuation drills, emergency supplies, and policies were inspected. The children were observed participating in circle time and engaging freely. Areas of non-compliance are identified in the Violation Notice. Based on observation and interview, the family day home that was built before 2015 and that serves preschool-age children was not equipped with at least one carbon monoxide detector.

Evidence:
1. The family day home was built before 2015.
2. The family day home was not equipped with at least one carbon monoxide detector.
3. The provider confirmed the family day home did not have a carbon monoxide detector on-site at the time of the inspection.
Plan of Correction: I will purchase it by Saturday, March 25, 2023.
Standard 8VAC20-800-100-A
Based on record review and interview, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Evidence: There was no documentation of a physical examination completed by or under the direction of a physician prior to or within 30 days of child #2's first day of attendance. The start date for child #2 is 5/8/2022.
Plan of Correction: I will talk to the parent today.
Standard 8VAC20-800-210-A
Based on record review and interview, caregivers did not obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

Evidence:
1. Between September 2021 and September 2022, caregivers #1 and #2 received 3 hours of annual training.

2. The provider confirmed that the annual training period for the family day home begins in September.

Evidence: Child #1 does not have signed documentation of viewing the proof of identity for the child. The start date for child #1 is 8/6/2022.

Based on record review and interview, before a child's first day of attendance, the provider did not obtain documentation that a child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations.

Evidence: There was no documentation that child #2 and child #5 had been adequately immunized prior to their first day of attendance, as required by Virginia Code ? 32.1-46 A. The start date for child #2 is 5/8/2022, and the start date for child #5 is 3/20/2023.
