Inspector Notes
An unannounced monitoring inspection was conducted on 3/22/2023 from 10:15 am-11:40 am. There were 8 children directly supervised by 2 caregivers. The home, 3 caregiver records, 5 children?s records, 2 household member records, evacuation drills, emergency supplies, and policies were inspected. The children were observed participating in circle time and engaging freely. Areas of non-compliance are identified in the Violation Notice. Violations
5Evidence:
1. The family day home was built before 2015.
2. The family day home was not equipped with at least one carbon monoxide detector.
3. The provider confirmed the family day home did not have a carbon monoxide detector on-site at the time of the inspection.
Evidence: There was no documentation of a physical examination completed by or under the direction of a physician prior to or within 30 days of child #2's first day of attendance. The start date for child #2 is 5/8/2022.
Evidence:
1. Between September 2021 and September 2022, caregivers #1 and #2 received 3 hours of annual training.
2. The provider confirmed that the annual training period for the family day home begins in September.
Evidence: Child #1 does not have signed documentation of viewing the proof of identity for the child. The start date for child #1 is 8/6/2022.
Evidence: There was no documentation that child #2 and child #5 had been adequately immunized prior to their first day of attendance, as required by Virginia Code ? 32.1-46 A. The start date for child #2 is 5/8/2022, and the start date for child #5 is 3/20/2023.
