Inspector Notes
An unannounced renewal inspection was conducted on 11/8/2023 from 8:20 am-9:35 am. There were 4 children supervised by 2 direct caregivers. The home, 2 caregivers records, 5 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed engaging freely. Areas of non-compliance are identified in the Violation Notice. Please submit your plan of correction within 5 business days for it to be included on the website. Violations
4Based on record review and interview, caregivers did not obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.
Evidence:
1. Between September 2022 and September 2023, training documentation showed that Caregivers #1 recieved 9 hours of annual training and Caregiver #2 received 5 hours of annual training.
2. The provider confirmed that the annual training period for the family day home begins in September.
Based on observation and interview, areas and furnishings of the family day home inside were not maintained in a clean, safe, and operable condition.
Evidence: In the playroom, there were several areas on the wall near the sliding glass door where the paint was chipping.
Based on observation and interview, the first aid kit did not contain all the required items.
Evidence: The first aid kit is missing adhesive tape and the first aid instruction manual.
Based on record review and interview, the provider did not annually review with the parent the emergency contact information to ensure the information was correct and obtain the parent?s signed acknowledgment of the review.
Evidence:
1. Child #3's last annual emergency contact review was on 8/9/2022.
2. Child #4's last annual emergency contact review was on 4/22/2022.
